Step 1: After purchasing one of our Class-2 Medical Devices, Download and Print out the appropriate FDA Certification Form and submit to your insurance company for approval.
Step 2: Make sure you include a prior Invoice obtained by calling our office at 385-645-0255 ext2
or emailing firstname.lastname@example.org with your square receipt.
Step 3: Submit all documents to your insurance carrier
